School and District Reports

Schools will receive

  • A frequency report within 6 weeks after survey administration that contains responses to all questions on the survey, presented by grade.

Individual school reports are made available directly to school principals through email for their own distribution.

 

To access your school's reports, click on the Login flag at the top of the page and select "School Report". You will be directed to a screen where you will enter the username and password emailed to you by the IYS Coordinator. Once on the report page, you will be able to open/download your school report(s).

 

School reports are confidential. They can be released directly by school administrators at their discretion or by CPRD with a school administrator's permission on the School Report Release Form. Providers or coalitions working with schools/districts and in need of frequency reports are strongly encouraged to ask school/district administrators directly to share their school/district reports with them.  There is no need to submit a School/District Report Release Form if you get frequency reports directly from school/district administrators. School Report Release forms must be renewed every administration year.

Districts will receive

  • A frequency report of surveys at all participating district schools that contains responses to all questions on the survey, presented by grade. This report is produced if the district contains more than one school at any eligible grade level.

District reports are made available directly to district administration through email for their own distribution. Single district reports are confidential and can only be released by CPRD with a district administrator's permission using the District Report Release Form. There is no need to submit a School/District Report Release Form if you get frequency reports directly from school/district administrators.
