If you just filled out an Illinois Youth Survey, we want to say "Thanks!" All surveys are anonymous. Surveys will only be used to learn about health behaviors of young people like yourself. This information will help your community, school, and the state of Illinois to better meet students’ needs.

If the survey bothered or upset you, or you would like help with an issue or crisis, we suggest you take one of these actions listed below:

  • Talk to an adult you trust in your family or community, such as your parents or religious leader.
  • Talk to an adult you trust at school, such as a teacher, counselor, nurse, intervention specialist, or principal.
  • Call one or more of the hotlines listed below to find out where to get help.
