Schools will receive
- A school frequency report within 6 weeks after survey administration that contains responses to all questions on the survey, presented by grade.

Individual school reports are made available directly to school principals though an online web portal for their own distribution.
To access your school report, click on the Login flag at the top of the page and select View Reports. You will be directed to a screen where you will enter the username and password emailed to you by the IYS Coordinator. Once on the report page, you will be able to open/download your school report(s).
Districts will receive
- A district frequency report of surveys at all participating schools in the district, including responses to all questions on the survey, presented by grade. This report is produced if the district has at least two schools that participated at any eligible grade level.
District reports are made available directly to district superintendents through an online web portal for their own distribution. School reports are also included on this portal for district use.
To access your district report, click on the Login flag at the top of the page and select View Reports. You will be directed to a screen where you will enter the district username and password emailed to you by the IYS Coordinator. Once on the report page, you will be able to open/download your district report(s) as well as the reports for participating schools in your district.

Information for Providers
- Providers or coalitions working with schools or districts and in need of frequency reports are strongly encouraged to ask school/district administrators directly to share their reports.
School reports are confidential. They can be released directly by school administrators at their discretion or by CPRD with a school administrator's permission on the School Report Release Form. There is no need to submit a School Report Release Form if you get school reports directly from school administrators. School Report Release Forms must be renewed every administration year.
Single district reports are confidential. They can be released directly by district administrators at their discretion or by CPRD with a district administrator's permission using the District Report Release Form. There is no need to submit a District Report Release Form if you get district reports directly from district administrators. District Report Release forms must be renewed every administration year.
