Schools Will Receive:
- A school frequency report within 6 weeks after survey administration that contains responses to all questions on the survey, presented by grade.
- Individual school reports are confidential and are only made available to school principals for their own distribution.
- School principals will receive a report login and password for the IYS Reports page.

Districts Will Receive:
- A district frequency report of surveys at all participating schools in the district, including responses to all questions on the survey, presented by grade. This report is produced if the at least two district schools participated at any eligible grade level.
- District reports are confidential and are only made available to district superintendents for their own distribution.
- District superintendents will receive a report login and password for the IYS Reports page.
- School reports for participating schools in each district are also available to superintendents using the IYS Reports page.
How to Access Your School / District Report
- Click on Login to Reports at the top right of the page.
- You will be directed to a screen where you will enter the username and password emailed to you by the IYS Coordinator.
- Once on the report page, you will be able to open/download your report(s).
- Both school and district logins provide access to reports for all years your school or district has participated in the IYS.
Report Release Forms
- School and district reports are confidential.
- School and District reports can be released directly by school or district administrators at their discretion or by CPRD with school administrator’s permission.
