Information for Providers
- Providers or coalitions working with schools/districts and in need of frequency reports are strongly encouraged to ask school/district administrators directly to share their school/district reports with them.
- There is no need to submit a School/District Report Release if you get frequency reports directly from school or district administrators.
School Report Release
- School reports are confidential.
- They can be released directly by school administrators at their discretion or by CPRD with school administrator’s permission on the School Report Release Form.
- There is no need to submit a School Report Release Form if you get school reports directly from school administrators.
- School Report Release Forms must be renewed every administration year.
District Report Release
- District reports are confidential
- They can be released directly by district administrators at their discretion or by CPRD with a district administrator’s permission using the District Report Release Form.
- There is no need to submit a District Release Form if you get district reports directly from district administrators.
- District Report Releases must be renewed every administration year.
Custom Analysis
- Custom analysis requests will be processed after the IYS data set is available and regular IYS reporting is finished, typically starting in January of the off year.
- Use the Custom Analysis Request Form if you are requesting:
- a standard IYS frequency report with a specific combination of schools, districts, or counties,
- any frequency report separated by other demographics, or
- something other than a frequency report (other statistical analysis).
